To register with any Pop Warner program you will need the following documentation:

1) Birth certificate. (original and a copy)

2) 2021 Pop Warner Participant Contract.

3) Copy of the child’s final 2019 – 2020 report card.

4) 2021 Pop Warner Physical Fitness & Medical History form.

5) Copy of your most recent utility bill to verify residency

 
Some associations require paperwork in addition to Pop Warner’s requirements such as.

1) Registration application unique to your association

2) Proof of medical insurance
